Health Center Assessment Tool:

This tool allows health center staff and leaders to reflect on the organization's progress in 15 change areas for transformation. It is intended to be completed by multiple staff across the organization, with results shared and discussed, so that teams can gain insights from multiple vantage points. Health centers can choose to share their averaged score with up to two PCAs or HCCNs. Step-by-step instructions are available to assist you in completing the assessment.

Health Center Partner (Coach) Assessment Tool:

This tool was created for staff working in state/regional/national partner organizations to reflect on their professional knowledge and skills in 15 change areas for transformation, and 2 coaching skills. It is intended to be completed individually to gain insights and potential areas for professional growth. PCA/HCCN staff can view their members’ health center scores, averaged by year, and overall progress, for health centers that elect to share.

PCA/HCCN/NTTAP staff can access step-by-step instructions for completing the assessment and instructions to view member health center data.

The Value Transformation Framework is an organizing framework to guide health center systems change and the translation of research, proven solutions, and promising practices into actionable steps.
