Newly-Released Form 5B Tools (From HRSA)

In early August, 2021, HRSA released the Form 5B Scope Accuracy Worksheet and the Instructions for Form 5B: Service Sites webpage.

These tools are intended to be used when completing new service site requests or other Form 5B changes. According to HRSA, updates include:

  • Clarification on which submission process in HRSA’s Electronic Handbooks (EHBs) a health center will use to update each field on Form 5B: (Formal Change In Scope (CIS), Scope Adjustment CIS, Self-update).

  • Simplified directions on recording suites, floors, or buildings on Form 5B.

  • Subrecipient language to more closely align with 45 CFR 75.

The 5B Scope Accuracy Worksheet was created to help facilitate scope accuracy conversations between health centers and their Project Officers. If you have an upcoming OSV, we recommend you review these documents and then contact your Project Officer to discuss together.
