Welcome to NACHC's Value Transformation Framework assessment portal.
This portal offers free, online tools designed to guide health centers in areas of system change fundamental to advancing on the Quintuple Aim using NACHC's Value Transformation Framework.
We currently offer:
Health Center (Organizational) Assessment (Elevate Participants Click Here): This tool allows health center staff and leaders to reflect on their organization's progress in 15 change areas for transformation. It is intended to be completed by multiple staff across the organization, with results shared and discussed, so that teams can gain insights from multiple vantage points. Health centers can choose to share their averaged score with up to two PCAs or HCCNs.

PCA/HCCN/NTTAP Staff Assessment (Elevate Participants Click Here): This tool was created for state/regional/national staff to reflect on their personal progress in 15 change areas for transformation, and 2 coaching skills. It is intended to be completed individually to gain insights and potential areas for improvement. PCA/HCCN staff can view their members health center scores, averaged by year, and overall progress, for health centers that elect to share.
